Root cause analysis.

چکیده
M unoz, Lowry, and Smith (2012) reported the use of root cause analysis (RCA) as part of a continuous quality improvement project to reduce hypoglycemic episodes in patients following gynecological surgery. Root cause analysis is an important component of quality improvement, and nurses at all levels should understand the process and the outcomes of a RCA. Root cause analysis was used originally in the automobile industry to improve production quality and efficiency (Sherwin, 2011). The process has been adapted to health care and is required by The Joint Commission (2011) when a sentinel event occurs. The process also can be used for errors or problems that are not sentinel events. In this column, I will provide a brief introduction to this key quality improvement activity.

Root Cause and Error Analysis
Error is an inevitable part of life and cannot be completely eliminated, but it can be minimized. A root cause analysis is a technique for understanding the systematic error causes that is involved beyond a person or people to implement an errors and including field and environmental causes of errors when occur in this situation too. An important factor of an error occurrence is a root cause (c...
متن کاملAnalysis of a possible root cause and mechanism for Soma mine disaster
A daunting mine disaster took place in 13 May 2014 at Soma and 301 men lost their lives. Brief information about the Eynez coal mine and some of the inherent characteristics of the field in terms of their effects on mining are presented. This paper basically concentrates on the factors that played an important role in the occurrence of this disaster. Progress of mine fire, firefighting, and res...
متن کاملA Root Cause Analysis of Haemolysis Encountered in Leuco-filtration of Stored Packed Red Cells Units
Background and Aims: Leukoreduction of blood components has reduced the incidence of transfusion-associated adverse events. Leucofiltration is the most effective method of leukoreduction. We encountered haemolysis in a series of leucofiltered units. This stressed our precious inventory, added to financial loss, increased our turn-around time to issue leucofiltered blood units, and placed doubts...
متن کاملRoot Cause Analysis of NIOC\'s 20" Natural Gas Explosion
Transmission and distribution pipelines have a significant role in the oil and gas industries. Pipelines are facing different threat like corrosion, improper design and incorrect operation. Gas vapor released from a pipeline (owned by NIOC) failure spread out and reached to ignition source. Due to gas outdoor spreading, “flash fire” occurred and flame return caused “vapor cloud explosion” of ga...
متن کاملA Practical Guideline Fora Successful Root Cause Failureanalysis
Root cause failure analysis is a process for identifying the true root cause of a particular failure and using that information to set a course for corrective/preventive action. From a technical standpoint, it is usually a multidisciplinary problem, typically focused on the traditional engineering fields such as chemistry, physics, materials, statics, dynamics, fluids, etc. However, it seems th...
متن کاملClustering of Event Sequences for Failure Root Cause Analysis
Root cause analysis is a critical part of the diagnostics expert system. The goal of root cause analysis is to allocate root cause by analyzing fault information with observed data. After gathering all data the two important questions that one needs to answer are: • Does the available data provide enough information to allocate root causes? • How to organize the data into meaningful structures ...
